Palliative Care Specialist Nurse

We are seeking an experienced Palliative Care Specialist Nurse to join our team at Lewisham and Greenwich NHS Trust. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care to patients and their families affected by life-limiting illnesses.
Key Responsibilities- Assess, plan, and provide specialist care and support to patients and their families.
- Participate in the management and triaging of new referrals and take part in daily allocation.
- Work collaboratively with colleagues, patients, carers, and other health and social care professionals to ensure optimum patient management.
- Act as a resource and advocate for patients and their families.
- Participate in audit and research to improve palliative care services.
- MSc level learning RN/RM with relevant specialist course or similar in palliative care.
- Advanced training/skills in invasive/therapeutic/diagnostic procedures relevant to speciality.
- Knowledge of conditions, management, and treatment of long-term conditions, including but not exclusively cancer, COPD, MND, and dementia.
- Senior clinical experience within the speciality of palliative and end-of-life care.
- Experience in practice guideline development, audit, and research.
- Ability to prioritise and organise workload, work autonomously, and as a member of a multi-professional team.
