About the Role

We are seeking a highly skilled and experienced Discharge Coordinator to join our team at Northern Lincolnshire and Goole NHS Foundation Trust. As a key member of our Integrated Discharge Team, you will play a vital role in ensuring the safe and efficient discharge of patients from our hospitals.

Key Responsibilities
  • Act as a facilitator and decision-maker, coordinating activity across a range of organisations to facilitate early safe discharge for hospital inpatients.
  • Provide clinical decision-making support as the point of contact and communication link between the hospital, locality team, and primary care.
  • Manage a complex caseload of patients requiring rehabilitation within the community and their own homes, including those at high risk of unplanned hospital admission.
  • Collaborate with experienced colleagues to support patients with complex health and social care packages, including those requiring discharge from hospital, fast track, and end-of-life discharges.

Person Specification

We are looking for a candidate with the following qualifications and experience:

  • ONMC, Social Work England, or HCPC Registered Health or Social Care professional.
  • Postgraduate Teaching/Mentorship qualification.
  • Postgraduate (level 7) accredited training relevant to specialist area of work.
