We are seeking a highly skilled and experienced Discharge Coordinator to join our team at Northern Lincolnshire and Goole NHS Foundation Trust. As a key member of our Integrated Discharge Team, you will play a vital role in ensuring the safe and effective discharge of patients from our hospitals.
Key Responsibilities- Act as a facilitator and decision-maker, coordinating activity across a range of organisations to ensure seamless patient care.
- Assist with the facilitation of early safe discharge for hospital inpatients, working closely with nursing, therapies, medical staffing, and local authority teams.
- Manage a clinical caseload, providing independent clinical decision-making and communication with hospital, locality team, and primary care services.
- Support hospital avoidance for patients requiring rehabilitation within the community and their own homes.
- Collaborate with experienced colleagues to deliver high-quality patient care, including patients at high risk of unplanned hospital admission, patients requiring discharge from hospital, and patients with complex health and social care packages.

Person SpecificationWe are looking for a candidate with the following qualifications and experience:
- ONMC, Social Work England, or HCPC Registered Health or Social Care professional.
- Postgraduate Teaching/Mentorship qualification.
- Postgraduate (level 7) accredited training relevant to specialist area of work.
- 3 years' experience of working at a senior level (Band 6 or equivalent).
- Evidence of continued professional development.
- Experience of independent clinical caseload management involving complex cases.
- Evidence of leadership, change, and people management.
