We are seeking a highly skilled and experienced Clinical Discharge Coordinator to join our team at Northern Lincolnshire and Goole NHS Foundation Trust. As a key member of our Integrated Discharge Team, you will play a vital role in ensuring the safe and timely discharge of patients from our hospitals.
Key Responsibilities:- Work closely with nursing, therapies, medical staffing, and local authority teams to identify pathways for medically optimized patients.
- Act as a facilitator and decision-maker, coordinating activity across a range of organizations to ensure seamless patient care.
- Assist with the facilitation of early safe discharge for hospital inpatients, working closely with community teams to support patients in their own homes.
- Manage the short-term care team to facilitate patients' step-down from acute care, providing support for hospital avoidance and community-based rehabilitation.
- Collaborate with experienced colleagues to support patients at high risk of unplanned hospital admission, including those in care homes, and patients requiring discharge from hospital, including fast-track and end-of-life discharges.
