Role Overview:

We are in search of a dedicated and empathetic Forensic Adult Safeguarding Practitioner to deliver specialized support to individuals facing mental health challenges within a forensic environment.

Your role will involve active participation in risk evaluations and the formulation of care pathways.

Adhering to the principles of the Recovery Model, you will integrate a social care perspective into patient care strategies.

You will take the initiative in addressing safeguarding concerns for both adults and children, collaborating closely with the Team Manager.


Key Responsibilities:

  • Conduct thorough nursing assessments and interventions for individuals within the forensic mental health framework, including those in correctional institutions and forensic psychiatric facilities.
  • Engage with multidisciplinary teams to create and execute personalized care plans that cater to the distinct needs of each individual.
  • Oversee discharge planning and transition services to ensure successful reintegration into the community.
  • Maintain precise and current clinical documentation in line with professional standards and regulatory guidelines.
  • Demonstrate a commitment to delivering exceptional patient care.
