Assistant Practitioner for Adult Safeguarding

About the Role:

We are seeking an experienced Assistant Practitioner to join our team in Adult Safeguarding. As an Assistant Practitioner, you will play a vital role in supporting vulnerable adults and their families, ensuring their safety and well-being.

Key Responsibilities:

  1. Conduct reviews and assessments to identify individuals' care and support needs, ensuring their plans are outcome-focused and relevant to their goals.
  2. Ensure statutory requirements are met for reviews, and undertake direct work with service users and their families/carers.
  3. Identify through supervision where cases require further assessment and re-allocation to a social worker.
  4. Promote access to paid employment, training, rehabilitation, social, and leisure opportunities to support individuals' independence and social inclusion.
  5. Identify risk and undertake risk assessments where safeguarding concerns are evidenced or suspected.

Requirements:

  1. Knowledge of the principles of empowerment and promoting independence.
  2. Knowledge of legislation, statutory guidance, and social work and care management practice.
  3. Knowledge of disabilities and their impact on individuals' lives.
  4. Experience of formulating and implementing support plans and reviewing their effectiveness.
  5. Experience of supporting service users to identify and realize their goals.
  6. Experience of safeguarding.
