Job Title: HIU Care Navigator

Job Summary:

The HIU Care Navigator will play a crucial role in providing tailored support to individuals identified as High Intensity Users (HIUs) of Urgent & Emergency Care services. This position requires a healthcare professional with excellent communication and interpersonal skills to coordinate and drive the delivery of services, ensuring improved health outcomes and reduced inequalities.

Key Responsibilities:

  • Identify HIUs who need additional support and review their needs with suitable clinical and other support
  • Refer patients to suitable providers via a local MDT as needed and track processes and outcomes
  • Publicize the service to users and staff across the Borough and work with a wide range of health, social care, and third sector organizations to prioritize care for this highly vulnerable group
  • Facilitate discussions between primary care, hospital, and community services to ensure coordinated and effective delivery of patient care

Requirements:

  • English and Maths GCSE
  • Specialist knowledge or interest acquired through training and extended courses related to healthcare
  • Proven record of excellent written and verbal communication and interpersonal skills
  • Good organizational and time management skills
  • Able to prioritize and manage own workload and show initiative to complete tasks independently and with multi-disciplinary teams

Working Arrangements:

This role is hybrid, with 4 days spent in practice and 1 day working remotely. The post holder will be required to work across several practices within the designated PCN structure.
