Healthcare Navigator

We are seeking a skilled and compassionate Care Navigator to join our team at NHS. As a key member of our primary care team, you will play a vital role in providing exceptional patient care and support.
Main Responsibilities:
- Process and signpost patients to the appropriate healthcare professional based on their presenting condition
- Answer incoming phone calls, transferring calls or dealing with caller requests appropriately
- Process patient requests for appointments and repeat prescriptions
- Initiate contact with patients, team members, and external agencies as needed
- Enter read-code data on Emis Web clinical system and maintain accurate patient records
Requirements:
- Educated to GCSE level or equivalent
- Active signposting or Care Navigator training qualification
- Healthcare qualification (level 2) or working towards gaining equivalent level
