Job Title: HIU Care Navigator

Job Summary:

We are seeking a highly skilled and motivated HIU Care Navigator to join our team at Healthcare Central London. The successful candidate will be responsible for coordinating and driving the delivery of our HIU service, working closely with healthcare professionals and other stakeholders to improve health outcomes and reduce inequalities.

Key Responsibilities:

  • Identify HIUs who need additional support and review their needs
  • Refer HIUs to suitable providers and track processes and outcomes
  • Facilitate discussions between primary care, hospital, and community services
  • Manage patient caseloads and advise on improvements
  • Provide information and training to local health and social care professionals

Requirements:

  • English and Maths GCSE
  • Specialist knowledge or interest acquired through training and extended courses related to healthcare
  • Proven record of excellent written and verbal communication and interpersonal skills
  • Good organisational and time management skills
  • Able to prioritise and manage own workload and show initiative to complete tasks independently and with multi-disciplinary teams

Working Arrangements:

The role is hybrid, with 4 days in practice and 1 day working remotely. The successful candidate will be required to work across several practices within the designated PCN structure.
