Key Responsibilities:

  • Support and coordinate care needs with clients, their natural support systems, and healthcare providers.">
  • Facilitate team meetings and ensure effective communication among all stakeholders.">
  • Document encounters and follow up activities in a timely manner.">
  • Ensure participants maintain eligibility for waiver programs and provide referrals for indicated services.">
  • Complete required documentation and meet program regulations.">
  • Assist with benefit management and ensure participants maintain active Medicaid.">

Requirements:

  • ">
  • Bachelor's degree in a Health and Human Service field or equivalent experience.">
  • Three years of experience providing care management services, preferably in a community-based setting.">
  • Knowledge of community resources, Medicaid services, and non-Medicaid services.">
  • Excellent communication and interpersonal skills.">
  • Able to work independently and as part of a collaborative team.">

Estimated Salary: $43,000 - $52,000 per year, depending on experience and qualifications. This figure is based on industry standards and market rates for similar positions in the New York area.
