Care Navigator

Job Summary:
The HIU Care Navigator will play a crucial role in supporting individuals who are high intensity users of urgent and emergency care services. This position requires a healthcare professional with excellent communication and interpersonal skills to coordinate and deliver tailored support to these individuals.
Key Responsibilities:
- Identify and review the needs of high intensity users, referring them to suitable providers as needed.
- Coordinate and track the delivery of services, ensuring effective and timely support.
- Work collaboratively with healthcare professionals, social care services, and third sector organizations to provide patient-centered care.
- Develop and maintain accurate records, using clinical systems and medical records to inform decision-making.
- Provide education and training to local health and social care professionals on the HIU service and its benefits.
Requirements:
- English and Maths GCSE.
- Specialist knowledge or interest acquired through training and extended courses related to healthcare.
- Proven record of excellent written and verbal communication and interpersonal skills.
- Good organisational and time management skills.
- Able to prioritise and manage own workload and show initiative to complete tasks independently and with multi-disciplinary teams.
Working Arrangements:
This role is hybrid, with 4 days spent in practice and 1 day working remotely. The post holder will be required to work across several practices within the designated PCN structure.
