About the Role

We are seeking a skilled and compassionate Mental Health Patient Navigator to join our team. As a key member of our healthcare team, you will provide exceptional support to patients, ensuring they receive the best possible care and guidance.

Key Responsibilities
  • Provide expert advice and guidance to patients on healthcare services and options available to them
  • Process and signpost patients to the appropriate healthcare professional based on their presenting condition
  • Answer incoming phone calls, transferring calls or dealing with caller requests appropriately
  • Process patient requests for appointments and repeat prescriptions
  • Initiate contact with and respond to requests from patients, team members, and external agencies
  • Enter read-code data on Emis Web clinical system and maintain accurate patient records
  • Support clinical staff with general tasks and maintain a clean and organized working area
Requirements
  • Educated to GCSE level or equivalent
  • Active signposting or Care Navigator training qualification
  • Healthcare qualification (level 2) or working towards gaining equivalent level
  • Experience of working in a primary care environment and with the general public
  • Excellent communication and interpersonal skills
