Mental Health Navigator

We are seeking a skilled and compassionate Mental Health Patient Navigator to join our team. As a key member of our healthcare team, you will provide exceptional support to patients, ensuring they receive the best possible care and guidance.
Key Responsibilities- Provide expert advice and guidance to patients on healthcare services and options available to them
- Process and signpost patients to the appropriate healthcare professional based on their presenting condition
- Answer incoming phone calls, transferring calls or dealing with caller requests appropriately
- Process patient requests for appointments and repeat prescriptions
- Initiate contact with and respond to requests from patients, team members, and external agencies
- Enter read-code data on Emis Web clinical system and maintain accurate patient records
- Support clinical staff with general tasks and maintain a clean and organized working area
- Educated to GCSE level or equivalent
- Active signposting or Care Navigator training qualification
- Healthcare qualification (level 2) or working towards gaining equivalent level
- Experience of working in a primary care environment and with the general public
- Excellent communication and interpersonal skills
