About the Role

We are seeking dedicated and compassionate individuals to join our team as Network Navigators in our Acute Mental Health Wards at Blossom Court, St Ann's Hospital. As a Network Navigator, you will play a vital role in ensuring that each patient's network of care is involved early in admission to inform care planning and timely discharge.

Key Responsibilities
  • Collaborate with patients and their networks to identify and address care needs
  • Develop and maintain relationships with community mental health and substance misuse teams, voluntary sector organizations, and other relevant services
  • Provide support and guidance to patients and their families to facilitate smooth transitions and care planning
Requirements
  • Diploma in Mental Health Practice or NVQ level 3 or 4 in Mental Health or equivalent experience
  • GCSE level or equivalent (with a minimum of 5 A - E qualifications - which must include Maths and English)
  • Digital competence: Outlook, Word, data entry in EPR systems, and Excel
