Job Summary:

We are seeking a highly skilled and compassionate Pediatric Nurse to join our team at Thornbury Community Services. As a Community Care Specialist, you will play a vital role in delivering exceptional care and support to young people in their own homes.

Key Responsibilities:

  • Provide high-quality, patient-centered care to children with complex needs in a community-based setting.
  • Work collaboratively with a multidisciplinary team to develop and implement individualized care plans.
  • Conduct thorough assessments and evaluations to identify the needs of each child and their family.
  • Develop and maintain strong relationships with families and caregivers to ensure seamless care transitions.
  • Participate in ongoing education and training to stay up-to-date on best practices and evidence-based care.

Requirements:

  • Minimum 12 months of experience as a Pediatric Nurse in the UK.
  • Strong knowledge of pediatric care principles and practices.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Valid UK nursing registration and a clean driving record.
