We are seeking a dedicated and proactive Healthcare Associate or Assistant Practitioner to join our Palliative Care Team. This role offers the chance to work in diverse acute and community environments, collaborating with a multidisciplinary team to evaluate, plan, and deliver clinical care and education tailored to service and patient requirements, under the guidance of supervising clinicians.

This position is an excellent opportunity to contribute to a growing service that supports Children and Young People, along with their families, who are facing life-limiting conditions and complex health challenges.

Key Responsibilities

  • Collaborate with the multidisciplinary team and Registered Nurse to assess, plan, and deliver clinical care and education based on service and patient needs.
  • Operate independently under the supervision of registered nurses, engaging in all aspects of nursing care.
  • Utilize knowledge and experience to make informed, evidence-based decisions in addressing challenges.
  • Recognize personal competency limits and maintain accountability for actions.
  • Supervise new starters and students during their placements after completing preceptorship and necessary training.
  • Actively participate in quality assurance and service development initiatives.
  • A valid driving license and access to a vehicle are required.

Job Duties

  • Provide compassionate and effective care to Children and Young People across various settings, ensuring adherence to care plans.
  • Collaborate with Registered Nurses in the ongoing assessment, planning, management, and evaluation of care.
  • Conduct and interpret clinical observations, including vital signs.
  • Perform clinical tasks such as catheterization and venepuncture, with additional competencies acquired through training.
  • Maintain accurate records of patients' nutritional and fluid intake as per care plans.
  • Ensure the privacy, dignity, and safety of all individuals at all times.
  • Recognize changing priorities and seek guidance from Registered Nurses or other healthcare professionals as necessary.
  • Uphold high standards of nursing care and report any concerns regarding care quality.
  • Administer medications in accordance with local policies and NMC Standards of Proficiency.
  • Identify changes in patients' conditions that require further intervention and refer to supervising Registered Nurses when necessary.
  • Execute delegated clinical and care tasks under the supervision of a Registered Nurse or other healthcare professionals.
  • Work independently when appropriate, delivering care according to defined care plans.
  • Be available for a variety of shifts, including nights and weekends.
  • Provide education in clinical skills to various staff members and stakeholders.

Qualifications and Experience

Essential

  • Willingness to complete competencies related to pediatric care.
  • Completion of Nursing Associate Foundation Degree or Assistant Practitioner degree.

Desirable

  • Supervisor training and experience.
  • Assessors course or equivalent.
