We are seeking a dedicated and proactive Nursing Associate or Clinical Support Practitioner to become a vital member of our Palliative Care Team.
In this role, you will engage in various acute and community environments, collaborating with a multidisciplinary team to evaluate, strategize, and implement clinical care and educational initiatives tailored to service and patient requirements, as directed by supervising healthcare professionals.
This position offers a unique chance to contribute to a growing service that supports Children and Young People facing life-limiting conditions and intricate health challenges.
Key Responsibilities
Collaborate with the multidisciplinary team and Registered Nurse to assess, plan, and deliver clinical care and education in accordance with patient needs. Operate independently under the guidance of registered nurses, adhering to all aspects of the nursing process. Maintain professionalism and utilize your expertise to make informed, evidence-based decisions to address challenges. Recognize and operate within your competency limits, taking responsibility for your actions. Supervise new team members and students during their placements, once you have completed your preceptorship and any additional training. Actively participate in quality assurance and service enhancement initiatives. Possess access to a vehicle and a valid driving license.About Sheffield Children's NHS Foundation Trust
