Job Description

Job Title: Band 6 Diabetes Specialist Nurse

Job Summary:

We are seeking an experienced Band 6 Specialist Nurse to join our Diabetes and Endocrine team at Chelsea and Westminster Hospital NHS Foundation Trust. The successful candidate will provide high-quality inpatient and outpatient diabetes care, working closely with a dynamic team to deliver excellent patient outcomes.

Main Responsibilities:

  1. Diabetes Care Provision: Provide skilled assessment, planning, delivery, and evaluation of specialist diabetes care within defined guidelines and protocols.
  2. Leadership and Education: Provide expert diabetes advice and guidance to nurses and other disciplines, and teach on nurse induction and mandatory update programs.
  3. Training and Development: Provide specialist training and education to own and other professions.
  4. Quality Improvement: Set and monitor standards of diabetes care and treatment, and participate in audit and research activity.
  5. Collaboration and Communication: Work as an integral member of the diabetes team, providing skilled professional leadership and collaborating with multidisciplinary teams to deliver high-quality patient care.
