About the Role

We are seeking a highly motivated and experienced Diabetes Specialist Nurse to join our dynamic and friendly Diabetes team for a 6 month fixed term contract.

The successful candidate will be responsible for delivering patient-centered advice, support, and care for patients with diabetes, and providing advanced assessment of patients' needs and advising on appropriate diabetes management.

The service delivers care in both the clinic and domiciliary setting, primarily to individuals with type 2 diabetes.

The successful applicant must have prior experience in caring for people with diabetes and be self-motivating, organized, computer literate, and able to demonstrate excellent interpersonal skills with a commitment to continued personal development.

Main Responsibilities
  • To promote the health and wellbeing of people with complex diabetes, enabling them to achieve optimum glycaemia control through informed self-management.
  • To empower and support people with diabetes to self-manage their condition, guiding them towards a 'normal' lifestyle and reducing morbidity and premature mortality by detecting and preventing complications.
  • To provide education and support to both people with diabetes and those who care for them.
  • To promote strong, effective, and regular collaborative working between all healthcare professionals and stakeholders along the diabetes care pathway.

  • Independent Nurse Prescriber or willingness to work towards
  • Experience of working with patients with complex diabetes
  • Experience of teaching patients/carers and healthcare professionals, formally and informally

  • Expert clinical skills in the field of diabetes
  • Expert knowledge of insulin and GLP-1 receptor agonist therapies
  • Able to demonstrate an understanding of the quality and governance agenda and actively participate in evaluation and improvement of quality within the service
  • IT literate, with an ability to analyze clinical data
