Job Summary

We are seeking a skilled and compassionate Diabetes Specialist Nurse to join our team at Healthcare Central London. As a federation of 33 General Practices in Westminster, we are dedicated to providing high-quality community and primary care services to our local residents.

Key Responsibilities
  • Provide comprehensive diabetes care to patients, including assessment, treatment, and ongoing management.
  • Develop and implement care plans for patients with diabetes, ensuring individualized and holistic care.
  • Collaborate with multidisciplinary teams to coordinate and optimize the delivery of diabetes care.
  • Educate patients on self-management techniques, lifestyle changes, and medication management.
  • Monitor and assess patients' glycemic control and recommend adjustments to treatment plans as necessary.
  • Support patients in adhering to prescribed treatment regimens and lifestyle modifications.
  • Act as a resource and provide guidance to healthcare professionals on the management of diabetes.
Requirements
  • Valid NMC Registration and post-graduate certification/qualification in Diabetes
  • Valid registration/qualification as a Nurse Independent Prescriber
  • Proven experience in managing and caring for patients with diabetes.
  • Sound knowledge of evidence-based guidelines and best practices in diabetes care.
  • Strong communication and interpersonal skills to effectively engage with patients and healthcare professionals.
  • Ability to work collaboratively in a multidisciplinary team environment.
  • Empathy and compassion for patients with chronic conditions.
  • Proficiency in using electronic health records and digital tools in diabetes management.
Benefits

Salary Range AfC Band 7 £51,488 - £57,802 pro-rata based on experience.
