Position Overview

Key Responsibilities

  • Provide operational support to neighbourhood nursing teams, ensuring effective management of daily functions and risk identification.
  • Foster the development of nursing staff to enhance patient outcomes and promote self-sufficiency, thereby reducing unnecessary hospital admissions.
  • Conduct comprehensive assessments and engage with patients and their families to facilitate education and self-care practices.
  • Ensure that nursing teams adhere to both national and local policies, delivering holistic, patient-centered care that is evidence-based.
  • Maintain clinical expertise to offer professional supervision and skill development to nursing teams.
  • Oversee all aspects of team leadership, development, and management within the designated locality.
  • Identify and eliminate barriers that hinder team performance and patient care.
  • Promote a positive image of neighbourhood nursing by collaborating with integrated teams and building strong relationships with local healthcare providers.
  • Effectively communicate and escalate any risks related to service delivery, staff, and patient care.
  • Be accountable for team performance and the quality of care provided.
  • Lead and motivate teams through visibility, approachability, and effective communication.

Qualifications and Experience

Essential

  • Registered nurse with an adult nursing degree and NMC registration.
  • Proficient in coaching methodologies and skilled in various techniques suitable for diverse contexts.
  • Experience collaborating with multiple organizations and the voluntary sector.
  • Knowledge of solution-focused interaction methods.
  • Demonstrated ability to support staff in complex clinical decision-making.
  • Mentorship qualification.
  • Experience in conflict resolution and team performance optimization.
  • Training in leadership and management.

Desirable

  • Qualification as a Non-Medical Prescriber/Nurse Prescriber.
