Job Summary:

We are seeking a skilled and dedicated nurse to join our Community Anticoagulation Service team. As a valued member of our team, you will be responsible for providing high-quality patient care and contributing to the development of our service.

Key Responsibilities:

  • Clinical Assessments: Conduct thorough clinical assessments of patients to ensure accurate and effective treatment plans.
  • Warfarin Clinics: Undertake warfarin clinics in various community settings, including patients' homes, to provide personalized care and education.
  • Team Collaboration: Work closely with a small and supportive team to influence all aspects of patient care and management.
  • Education and Training: Participate in education and training programs to enhance your skills and knowledge in anticoagulation management.
  • Quality Control: Contribute to the quality control process by participating in NEQAS external quality control scheme for near-patient testing analyzers.
  • Phlebotomy: Perform phlebotomy when required to ensure accurate blood sampling and testing.
  • Service Development: Collaborate with the team to develop and implement new ideas and initiatives to improve patient care and service delivery.

Requirements:

  • Qualifications: Registered Nurse with a valid nursing license.
  • Experience: Previous experience in a community or hospital setting, preferably in anticoagulation management.
  • Skills: Strong clinical skills, excellent communication and interpersonal skills, and ability to work independently and as part of a team.
