We are seeking a skilled and compassionate Registered Nurse to join our Medway Community Anticoagulation Service team. As a key member of our team, you will play a vital role in providing high-quality patient care and contributing to the delivery of excellent clinical outcomes.
Main Responsibilities- Warfarin Clinics: You will undertake warfarin clinics in various community settings, including patients' homes, and work closely with a small and supportive team to ensure seamless care delivery.
- Patient Education: You will participate in new patient education clinics, providing one-to-one education sessions to patients newly started on warfarin, and train patients on the use of CoaguChek point-of-care test devices.
- Phlebotomy and Testing: You will perform phlebotomy when required and participate in the NEQAS external quality control scheme for near-patient testing analysers.
- Service Development: You will contribute to the ongoing development and improvement of our service, including participating in regular review and rewriting of patient information leaflets.
- Registered Nurse: You must be a registered nurse with a valid nursing license.
- Community Experience: You should have experience working in a community setting, preferably in a hospital or community background.
- Educated to Degree Level: You must be educated to degree level or equivalent.
- Anticoagulation Qualification: You should have an anticoagulation qualification or be willing to undertake one.
- Caring and Compassionate: You must demonstrate a caring and compassionate approach to patient care.
- Delivering Quality and Value: You must be committed to delivering high-quality patient care and contributing to the delivery of excellent clinical outcomes.
- Working in Partnership: You must be able to work effectively in partnership with patients, families, and other healthcare professionals.
