Job Summary

An exciting opportunity has arisen for a skilled nurse to join our Medway Community Healthcare team as a Staff Nurse in our Anticoagulation Service. As a valued member of our team, you will be responsible for providing high-quality patient care and contributing to the development of our service.

Main Responsibilities
  • Conduct clinical assessments of patients and provide individualized care plans.
  • Participate in the computerized dosing of patients and liaise with medical staff and nursing colleagues as needed.
  • Deal with clinical problems and escalate to medical staff or nursing line manager as required.
  • Provide a telephone and email advisory service to patients regarding their anticoagulation treatment.
  • Participate in new patient education clinics and provide one-to-one education sessions for patients starting warfarin.
  • Train and become competent in the use of coaguchek point of care test devices and participate in finger prick clinics.
  • Participate in NEQAS external quality control scheme for near patient testing analysers.
  • Perform phlebotomy as required.
