Job Summary

We are seeking an experienced and compassionate leader to join our team as a Neighbourhood Nursing Team Lead. As a key member of our community healthcare team, you will be responsible for supporting our neighbourhood nursing teams to deliver high-quality, patient-centered care to our local population.

Key Responsibilities
  • Support teams with the day-to-day operational and functional aspects of neighbourhood nursing, identifying and managing risks with the support of the Clinical Lead and Head of Service.
  • Develop and support neighbourhood nurses to achieve the best outcomes for patients and their families, and teams to be able to function optimally and as self-sufficiently as possible.
  • Support holistic assessments and engagement with patients and families in education to support self-care where appropriate.
  • Ensure the neighbourhood nursing teams deliver services in line with national and local policies, providing holistic, patient-centered, high-quality, evidence-based care to our local population.
  • Maintain clinical competence and credibility to enable provision of both professional and clinical supervision, skill development, and support to the neighbourhood nursing teams as required.
  • Lead, manage, and motivate teams by being highly visible, approachable, and communicative at all levels.
  • Develop teams and capacity through removing blocks and barriers that impact negatively on enabling teams to care.
  • Escalate appropriately risks to service provision, staff, and patients.
  • Be accountable to the Clinical Lead for teams' performance and quality of care of service delivery.
Requirements
  • Adult nursing degree and NMC registration.
  • Knowledge and understanding of coaching processes, as well as the variety of styles, skills, and techniques that are appropriate to the context in which the coaching takes place.
  • Experience of working with multiple organisations and voluntary sector.
  • Knowledge of Solution Driven Method of Interaction.
  • Is an experienced practitioner able to undertake and support staff in the workplace with complex clinical decision making.
  • Mentorship qualification.
  • Experience of managing conflict resolution and working with teams to optimise performance.
  • Leadership and management training.
