Job Title: Community Nurse Lead

We are seeking an experienced Community Nurse Lead to join our team. As a Community Nurse Lead, you will be responsible for supporting our neighbourhood nursing teams to achieve the best outcomes for our patients and their families.

Key Responsibilities:
  • Support teams with the day-to-day operational and functional aspects of neighbourhood nursing, identifying and managing risks with the support of the Clinical Lead and Head of Service.
  • Develop and support neighbourhood nurses to achieve the best outcomes for patients and their families, and teams to function optimally and as self-sufficiently as possible.
  • Support holistic assessments and engagement with patients and families in education to support self-care where appropriate.
  • Ensure neighbourhood nursing teams deliver services in line with national and local policies, providing holistic, patient-centred, high-quality evidence-based care to our local population.
  • Maintain clinical competence and credibility to enable provision of both professional and clinical supervision, skill development, and support to neighbourhood nursing teams as required.
Requirements:
  • Adult nursing degree and NMC registration.
  • Knowledge and understanding of coaching processes, including persuasion, motivation, and reassurance.
  • Experience of working with multiple organisations and voluntary sector.
  • Knowledge of Solution Driven Method of Interaction.
  • Leadership and management training.
  • Non-Medical Prescriber/Nurse Prescriber.
