Job Summary:
We are seeking a skilled and dedicated nurse to join our Community Anticoagulation Service team. As a valued member of our team, you will be responsible for providing high-quality patient care and contributing to the development of our service.
Key Responsibilities:
- Clinical Assessments: Conduct thorough clinical assessments of patients to ensure accurate and effective treatment plans.
- Warfarin Clinics: Undertake warfarin clinics in various community settings, including patients' homes, to provide convenient and accessible care.
- Team Collaboration: Work closely with a small and supportive team to share knowledge, expertise, and best practices.
- Patient Education: Provide one-to-one patient education sessions to empower patients with the knowledge and skills needed to manage their anticoagulation treatment.
- Quality Control: Participate in the NEQAS external quality control scheme to ensure the accuracy and reliability of our near-patient testing analysers.
- Phlebotomy: Perform phlebotomy when required to support patient care.
- Service Development: Contribute to the development and improvement of our service, including the creation of patient information leaflets and the exploration of new ways of working.
Requirements:
- Full Driving Licence: Possess a full driving licence and have access to a car.
- Previous Experience: Previous experience in an anticoagulation setting is not required, as comprehensive training will be provided.
- Communication Skills: Excellent communication and interpersonal skills to effectively interact with patients, colleagues, and medical staff.
