About the Role
Location: Community-based with flexible working arrangements.
Contract: 21 hours per week.
As a Dementia Support Specialist, you will have the rewarding opportunity to provide expert support, guidance, and resources to individuals with dementia and their caregivers, empowering them to maintain their independence and improve their overall well-being.
The service is tailored to each individual's unique needs and circumstances, and you will offer support through various channels, including in-person visits, phone calls, and digital communication.
You will also assist individuals in accessing relevant services, providing signposting and referrals, and delivering group programs within the community to foster support networks and friendships.
Your key responsibilities will include:
- Providing expert support and guidance to individuals with dementia and their caregivers.
- Improving individuals' sense of well-being and enabling them to take control of their lives.
- Assisting with the identification of needs and providing access to relevant services.
- Promoting the Dementia Advice service and building relationships with healthcare professionals.
About You
We are seeking an individual with:
- A passion for supporting individuals with dementia and their caregivers.
- Excellent communication and interpersonal skills, with the ability to adapt to diverse needs and circumstances.
- Experience working with individuals with complex needs and supporting them to access relevant services.
- Strong networking and collaboration skills, with the ability to work effectively with healthcare professionals.
- Good IT skills and experience working with digital platforms.
