Patient Flow Coordinator

Peterborough, Peterborough, United Kingdom North West Anglia NHS Foundation Trust Full time

Job Overview

The Capacity Manager plays a crucial role in overseeing the daily operations that ensure the effective management of patient flow throughout the hospital's medical and surgical departments. This position is essential for facilitating a seamless patient experience, aligning with the Trust's strategic and operational goals.

In collaboration with medical and surgical teams, as well as specialty managers, the Capacity Manager will forecast patient admissions and respond to urgent care pathways, including Emergency and Family Integrated Support Services (FISS).

The role involves direct operational oversight of capacity management, ensuring that all processes run smoothly while managing the Patient Flow Coordinator team and supporting the Site Senior Manager in meeting both local and national standards.

This position requires support for all staff, working both during and outside of regular hours to guarantee the safe and efficient use of resources, ultimately delivering high-quality patient care. The Capacity Manager operates independently, with the Site Senior Manager available for timely escalation and resolution of issues, maintaining the integrity of acute services.

Key Responsibilities

The Capacity Manager is pivotal in achieving the Emergency Department target of 95% of patients being seen, treated, admitted, or discharged within a four-hour timeframe. This is accomplished by focusing on minimizing patient pathway delays and intervening as necessary to uphold patient safety and care quality.

Responsibilities may include proactive measures to prevent delays, such as coordinating results, transport, and discharge plans. The Capacity Manager may also assess patient well-being, leveraging clinical knowledge and experience while collaborating closely with clinicians and ward teams to achieve optimal outcomes in complex scenarios.

With a deep understanding of NHS patient pathways, the Capacity Manager supports and acts as a deputy for the Site Senior Manager, working in conjunction with senior nursing and management teams to ensure targets are met and escalated when necessary.

The Capacity Manager will work across all divisions to ensure that available capacity is utilized effectively, maintaining and enhancing patient flow and safety. This includes adhering to Infection Prevention and Control protocols, particularly in light of Covid-19 and other hospital-acquired infections that necessitate isolation measures, which significantly affect patient flow.
