Community Nursing Coordinator Band 6

We are pleased to present a rewarding opportunity for a dedicated professional to join our Community Nursing Division as a Community Nursing Coordinator. This role involves working collaboratively within a dynamic team to deliver exceptional care to patients in both home and clinic environments.

Key Responsibilities

This is a permanent position with flexible hours to be discussed during the interview process.

The primary responsibility of the Community Nursing Coordinator is to oversee and manage tailored care plans for patients, ensuring high-quality nursing care is delivered effectively and efficiently. This role requires collaboration with various health and social care professionals, as well as voluntary and statutory organizations.

Job Duties

The successful candidate will be expected to work independently as well as part of a cohesive team. Strong time management, organizational, and communication skills are essential. The role requires adaptability and a willingness to respond to the evolving needs of the organization.

Access to a vehicle for work-related travel is a requirement.

Qualifications and Professional Training

Essential

  • Registered General Nurse with a degree or equivalent qualification
  • Current registration with the NMC
  • Specialist Practitioner Qualification in District Nursing or equivalent
  • Evidence of recent professional development

Desirable

  • Qualification as an independent non-medical prescriber or willingness to pursue
  • Teaching or mentorship qualification
  • Clinical diagnostics and examination modules (or willingness to undertake)
  • Leadership qualification (ILM level 6 or equivalent) or relevant experience

Experience and Knowledge

Essential

  • Proven experience in managing a caseload within a community setting
  • Knowledge of managing patients with chronic conditions
  • Understanding of tissue viability and management of pressure and leg ulcers
  • Experience in end-of-life care
  • Familiarity with community nursing practices
  • Awareness of workforce policies and procedures
  • Experience in teaching and mentoring
  • Ability to work in a multidisciplinary team
  • Supervisory experience or clinical supervision
  • Understanding of professional issues and developments
  • Knowledge of national health and social care initiatives
  • Skills in clinical assessment, planning, and evaluation
  • Capability to manage complex cases
