Position Overview

We are thrilled to announce a unique opportunity to become a pivotal member of our innovative and expanding Frailty Virtual Ward team. We seek a seasoned and dedicated senior nurse (Band 7) who is enthusiastic about enhancing service delivery and ensuring exceptional patient-centered care. This role involves close collaboration with a Multi-Disciplinary Team comprising Doctors, senior nurse practitioners, Occupational Therapists, Physiotherapists, and Healthcare Assistants, all committed to fostering independence, encouraging self-care, and supporting patients in their home environments. A key aspect of this position is to provide strong leadership and maintain a credible nursing presence that is accessible to staff, patients, and their families. The role may occasionally require autonomous work while engaging with patients in their homes and within hospital settings.

Key Responsibilities

- Deliver specialized, high-quality care and support to patients in their residences to avert unnecessary hospital admissions and facilitate timely discharges from various healthcare settings, under the direct or indirect supervision of senior medical staff.
- Operate at an advanced clinical level, making critical decisions based on comprehensive clinical assessments within the community.
- Provide clinical leadership and guidance within the Frailty Virtual Ward nursing/AHP team, focusing on service development to enhance patient experiences and clinical outcomes while ensuring patients remain in their homes.
- Oversee operational continuity, financial management, and strategic development of the Frailty Virtual Ward.
- Maintain effective communication with all key stakeholders, including General Practitioners, social services, acute hospitals, care providers, and voluntary organizations.
- Monitor current team operations and assist the service management team in delivering effective services that meet Key Performance Indicators (KPIs), Trust objectives, and both local and national standards while progressing in line with emerging initiatives.

Qualifications and Skills

Essential
- NMC Registered Nurse
- Independent Non-Medical Prescriber Qualification & registration with NMC
- Educated to Master’s Level or equivalent experience
Skills/Abilities
- Highly specialized clinical and technical skills in managing various clinical conditions and end-of-life care
- Strong analytical and judgment skills, with the ability to interpret complex situations and make informed decisions
- Excellent interpersonal skills and the ability to cultivate and maintain relationships with other health and social care professionals, patients, and their families
- Capacity to act as a clinical leader, effectively prioritizing personal and team workloads
- Demonstrated effective performance management capabilities
Experience
- Extensive post-registration experience in a Band 7 role
- Experience working within a multidisciplinary team
- Proven track record of engaging with key stakeholders and external partners
- Experience in managing staff and resources
- Involvement in Quality Improvement Projects
Knowledge
- Understanding of healthcare within an inner-city, culturally diverse community
- Familiarity with Quality and Risk agendas
- Experience managing a clinical environment
