Job Overview

We are thrilled to announce an opportunity to become a vital member of our pioneering and enthusiastic team within the expanding Frailty Virtual Ward service. We seek a dedicated and skilled senior nurse (Band 7) who is committed to enhancing service delivery and ensuring exceptional patient-centered care. In this role, you will collaborate closely with a Multi-Disciplinary Team comprising Doctors, senior nurse practitioners, Occupational Therapists, Physiotherapists, and Healthcare Assistants, all dedicated to fostering independence, encouraging self-care, and supporting patients in their own homes. This position may occasionally require you to operate independently while providing care across various settings, including patients' residences and inpatient facilities.

Key Responsibilities

o The successful candidate will engage in a multidisciplinary approach to deliver care to patients, assessing, planning, implementing, and evaluating their healthcare needs in both hospital and home environments. You will work within the Frailty Virtual Ward to avert unnecessary hospital admissions and/or facilitate timely discharges from healthcare facilities under the direct or indirect supervision of senior medical staff. You will be responsible for providing a high standard of comprehensive nursing care while promoting health education and maintaining wellness. Participate in the development of the Frailty Virtual Ward service, including audits and outcome measures, to instill a culture of continuous improvement and achieve positive patient outcomes. Engage with patients facing complex social challenges and those from diverse cultural backgrounds in a demanding socioeconomic environment. In the absence of the Band 8A Frailty Virtual Ward Lead, you will take on the responsibility of operationally managing designated areas of the service.

Education and Qualifications

Essential

  • Registered General Nurse (NMC Registered)
  • Evidence of ongoing post-registration training.

Desirable

  • Advanced Physical Assessment Course or willingness to undertake
  • Educated to master's level or equivalent experience
  • Non-Medical Prescribing (willingness to undertake)

Skills and Abilities

Essential

  • Experience managing the complex needs of patients in various settings
  • Phlebotomy skills
  • Cannulation
  • Ability to identify a deteriorating patient (Sepsis, mental health crisis, self-neglect, end of life)
  • Competence in assessing and treating various wounds (surgical, traumatic, pressure ulcer, leg ulcer, moisture lesions)
  • Capacity to work independently.

Experience

Essential

  • Post-registration experience as a Band 6
  • Experience in identifying, reporting, managing, and escalating safeguarding concerns
  • Experience in conducting first contact assessments in the community

Knowledge

Essential

  • Understanding of the Mental Capacity Act 2005 and ability to complete MCAs, best interest decision-making
  • Knowledge of clinical governance and ensuring team compliance
  • Awareness of professional accountability and delegation issues
