About the Role

We are seeking a highly skilled and compassionate individual to join our Home Treatment and Rapid Response team as a Support Specialist. As a key member of our team, you will play a vital role in providing high-quality support to individuals and their families in their own homes, promoting recovery and reducing the need for hospital admission.

Key Responsibilities
  • Contribute to the assessment and development of crisis plans for individuals referred to our service.
  • Provide support and guidance to individuals and their carers, promoting independence and well-being.
  • Work collaboratively with other healthcare professionals to ensure seamless care and support.
  • Monitor progress and adjust crisis plans as needed, ensuring the best possible outcomes for our service users.
  • Communicate effectively with senior staff and other stakeholders to ensure effective service delivery.
