Role Overview

We are seeking a dedicated individual to join our team as a Mental Health Support Worker specializing in Home Treatment and Rapid Response. In this role, you will be responsible for delivering client-centered care to patients and their families during crisis situations, providing essential support and interventions to help them navigate challenging times.

Key Responsibilities

  • Conduct comprehensive assessments of clients referred to the Home Treatment and Rapid Response program.
  • Provide personalized care, daily living assistance, and social engagement opportunities for clients and their caregivers.
  • Implement crisis intervention plans and ensure they are regularly updated as needed.
  • Monitor client progress closely and communicate your findings to senior staff members.
  • Respond promptly to crisis situations and adjust priorities accordingly.
  • Engage clients and their families in crisis prevention and recovery strategies.
  • Support clients in maintaining their independence at home and administer medication as required.
  • Promote the services of Home Treatment and Rapid Response to relevant stakeholders.

Essential Requirements

  • A genuine willingness to pursue relevant qualifications.
  • Possess knowledge and skills appropriate for a support worker role within mental health services.
  • Previous experience working in mental health settings is essential.
  • Experience handling emotional distress and navigating the complexities of mental health services is highly desirable.
