Position Overview:
As a Child Welfare Services Coordinator, you will be engaged in a pivotal role within the healthcare sector, focusing on the management of hospital discharge processes.
Key Responsibilities:
  • Lead a comprehensive city-wide initiative aimed at facilitating smooth transitions from hospital to home.
  • Collaborate with various stakeholders to minimize hospital stay durations and advocate for a home-first approach.
  • Contribute to the enhancement of the Transfer of Care Hub and associated pathways.
  • Engage with service users, caregivers, and multidisciplinary teams to design and implement effective reablement and strength-based services.
  • Ensure that the services provided are aligned with the goal of improving individual outcomes while maximizing value.
Work Environment:
This position offers flexibility, requiring in-office attendance for two days each week.
