About the Role

Ephesians Health Care is seeking a skilled Care Coordinator Associate to join our team. As a key member of our care management team, you will be responsible for coordinating and delivering high-quality care services to our clients.

Key Responsibilities
  • Client Assessments: Conduct thorough assessments to determine the needs of our clients and develop personalized care plans.
  • Care Service Coordination: Coordinate and monitor the delivery of care services to ensure our clients' needs are met.
  • Client Support: Provide guidance and support to our clients and their families to ensure they receive the best possible care.
  • Collaboration: Collaborate with healthcare professionals to ensure our clients receive comprehensive care.
  • Record Keeping: Maintain accurate and up-to-date client records.
  • Evaluation and Improvement: Monitor and evaluate the effectiveness of care plans and make adjustments as needed.
  • Client Advocacy: Advocate for our clients and ensure their rights are protected.
Requirements
  • Education: Bachelor's degree in a related field, such as nursing, social work, or psychology.
  • Experience: Previous experience in a care management or similar role.
  • Skills: Strong communication and interpersonal skills, ability to assess client needs, and knowledge of community resources and support services.
  • Organizational Skills: Excellent organizational skills and attention to detail.
  • Teamwork: Ability to work independently and as part of a team.
What We Offer
  • Competitive Pay: £12.50-£14.00 per hour.
