Physiotherapist - Stroke Rehabilitation Specialist

We are seeking a highly skilled and motivated Band 6 Physiotherapist to join our Community Stroke Rehabilitation Team. As a key member of the team, you will be responsible for delivering high-quality physiotherapy care to patients with stroke-related conditions in the community.
Main Responsibilities- Conduct comprehensive physiotherapy assessments of patients with diverse presentations and complex physical and psychological conditions following a stroke.
- Develop and deliver individualized treatment programs to meet the unique needs of each patient.
- Work collaboratively with the multidisciplinary team to provide a comprehensive package of care to stroke patients and their carers.
- Participate in evidence-based audit and research projects to enhance clinical practice and contribute to the implementation of changes to practice or service protocols.
- Contribute to the delivery of health promotion activities within the service and wider Trust initiatives.

Requirements- Recognized Diploma/Degree in Physiotherapy.
- Evidence of ongoing CPD in portfolio.
- Ability to prioritize, organize, and work in a busy environment.
- Good communication skills, able to present information (written and oral) in a clear and logical manner.
- Competent IT skills.
