We are seeking an experienced Learning Disability Nurse to join our team at Pennine Care NHS FT. As a key member of our multidisciplinary team, you will provide specialist health and behaviour assessments to individuals referred to our learning disability team.

Key Responsibilities
  • Deliver comprehensive assessments for individuals with learning disabilities and complex needs.
  • Develop and implement interventions and specialized care plans.
  • Work in partnership with social services and other stakeholders to ensure responsive, effective, and person-centered services.
  • Contribute to the development of policies and procedures.
  • Participate in local and regional specialist networks.

  • Experience in partnership working and managing risk.
