  • Up-to-date safeguarding knowledge, with a clear understanding of your own and your team's responsibilities.
  • Implement systems and processes to monitor and evaluate your team's progress against key performance indicators.
  • Effectively manage any human resources issues that may arise.

About the Role

You will oversee a team of both qualified and unqualified practitioners. Your team will manage a designated caseload that includes complex and high-risk cases. They will conduct strength-based Adult Needs Assessments and Reviews, collaborating with service users and their families to enhance independence while assessing and managing identified risks, considering any informal support available.

The Learning Disability team is dedicated to delivering high-quality service and is passionate about providing the best support for individuals accessing our specialized services.

Our committed team includes a Lead Practitioner, Qualified Practitioners (Nurses and Social Workers), and Community Care Officers. Each practitioner brings a wealth of knowledge and skills, offering support and nurturing, with unique qualities contributing to the team's success.
