About This Role

As a Community Health Care Support Worker, you will play a vital role in delivering compassionate and effective care to patients within their homes and clinical settings.   • Maintain accurate and up-to-date patient records, documenting observations and interventions effectively.
  • Communicate effectively with patients, families, and healthcare team members, fostering open and transparent communication.
