Job Title: Reablement and Care Support Worker

About the Role:

We are seeking a compassionate and dedicated individual to join our team as a Reablement and Care Support Worker. As a key member of our team, you will play a vital role in providing care and support to residents in Powys, enabling them to live independently in their own homes.

Key Responsibilities:

  • Provide person-centered care and support to residents, promoting their independence and well-being.
  • Work collaboratively with colleagues and other stakeholders to deliver high-quality care services.
  • Conduct thorough assessments of residents' needs and develop personalized care plans.
  • Deliver care and support services in a timely and efficient manner, ensuring residents receive the best possible outcomes.
  • Maintain accurate records and reports, adhering to Powys County Council's policies and procedures.
