Position Overview

  • The successful candidate will engage in various specialty functions within Acute Medicine, focusing primarily on Acute Hospital at Home (AHAH) and Outpatient Parenteral Antimicrobial Therapy (OPAT), while also participating in Same Day Emergency Care (SDEC).
  • In the AHAH capacity, the nurse will spearhead the seamless transition of care between the acute trust and community settings, ensuring patient safety and the delivery of high-quality care. Current patient streams managed include OPAT, AF, COVID, AKI, Heart Failure, NSTEMI, Syncope, and Frailty, with plans to expand services.
  • The nurse will actively identify and recruit new patients from the Medical Assessment Unit (MAU), SDEC, and hospital wards, conducting assessments to facilitate safe discharges into AHAH.
  • Continuous improvement and maintenance of the AHAH service will be based on best practices and current national guidelines.
  • The role requires ongoing professional development, including the completion of an advanced practice module pertinent to their responsibilities, such as history taking and clinical examination.
  • Providing continuous professional and clinical leadership to the care team is essential, delivering evidence-based care as part of a broader multidisciplinary team.
  • The nurse is expected to perform all relevant care and procedures competently and independently.

The Acute Medicine Specialist Nurse position has been established to address the increasing demands of the local community for a diverse multidisciplinary team. This role encompasses various areas and modalities, with the potential to complete an advanced practice module after a suitable period in the position.

Virtual wards offer a safe and effective alternative to traditional NHS bedded care, allowing patients who would typically require hospitalization to receive necessary acute care and treatment in their homes. This approach aims to prevent unnecessary admissions and support timely discharges.

The integration of digitally enabled care, including virtual wards, is a national priority reflected in current health and social care policies.

This position, as part of a multidisciplinary team, offers the opportunity to drive systemic transformation by incorporating digital technology to alleviate pressure on acute care services and enhance patient outcomes.

Work Schedule: The role requires flexibility, including working days, weekends, and bank holidays.
