Position Overview

  • The successful candidate will engage in various specialized Acute Medicine roles, focusing primarily on Acute Hospital at Home (AHAH) and Outpatient Parenteral Antimicrobial Therapy (OPAT), while also participating in Same Day Emergency Care (SDEC).
  • In the AHAH capacity, the individual will spearhead the smooth transition of care from the acute trust to community settings, ensuring patient safety and optimal care delivery. Current patient categories managed by this service include OPAT, AF, COVID, AKI, Heart Failure, NSTEMI, Syncope, and Frailty, with plans to expand into new areas.
  • The role involves actively identifying and recruiting new patients from the Medical Assessment Unit (MAU), SDEC, and hospital wards, assessing their needs, and facilitating their safe discharge into AHAH.
  • Continuous improvement and maintenance of the AHAH service will be expected, adhering to best practices and current national guidelines.
  • The candidate will be required to pursue ongoing education and complete an advanced practice module pertinent to their responsibilities, including skills in history taking and clinical examination.
  • Providing ongoing professional and clinical guidance to the care team and delivering evidence-based care as part of a broader multidisciplinary team is essential.
  • The individual is expected to perform all relevant care procedures competently and independently, without direct supervision.

This role presents a unique chance for a Registered Nurse to contribute their expertise to our integrated Virtual Ward. The position of Specialist Nurse in Acute Medicine has been established to address the increasing demands of the local community for a versatile multidisciplinary team. The role encompasses working across various areas and modalities, with the potential to complete an advanced practice module after a suitable period in the position.

The Virtual Ward serves as a safe and effective alternative to traditional NHS inpatient care, allowing patients who would typically require hospitalization to receive necessary acute care and treatment in their own homes. This approach aims to prevent unnecessary hospital admissions and support timely discharges.

The integration of digitally enabled care, including virtual wards, is a national priority embedded within contemporary health and social care policies.

This position, as part of a multidisciplinary team, offers an opportunity to drive transformation within the healthcare system by incorporating digital technology to alleviate pressure on acute care services and enhance patient outcomes.
