Job Description

We are seeking a highly skilled and compassionate Senior Support Worker to join our team at Medway Community Healthcare. As a key member of our care team, you will be responsible for providing high-quality care and support to our residents with dementia.

Key Responsibilities:
  • Leadership and Management: Supervise and support a team of support workers to ensure the delivery of excellent care and services.
  • Care Planning and Delivery: Develop and implement individualized care plans that meet the unique needs and goals of our residents.
  • Communication and Relationship Building: Build and maintain strong relationships with residents, families, and other healthcare professionals to ensure seamless care coordination.
  • Training and Development: Participate in ongoing training and professional development to stay up-to-date on best practices in dementia care and community healthcare.
  • Quality Assurance: Contribute to quality assurance initiatives to ensure the highest standards of care and service delivery.
Requirements:
  • Education and Experience: Foundation degree in health and social care, with a minimum of 2 years of experience in dementia care.
  • Skills and Knowledge: Excellent communication and interpersonal skills, with the ability to work effectively in a team environment. Strong knowledge of dementia care principles and practices, with the ability to apply this knowledge in a community healthcare setting.
  • Personal Qualities: Compassionate, empathetic, and patient, with a strong commitment to delivering high-quality care and services.
