About the Position:

Lead and coordinate a solution-focused strategy, engaging individuals in suitable services to transition them away from homelessness. Identify and connect with individuals experiencing homelessness and those in need, providing immediate assistance through outreach efforts at various times and locations, including those hesitant to engage with support services. Manage a caseload of individuals with intricate needs, creating tailored action plans aimed at reducing instances of homelessness. Collaborate in multi-agency initiatives to mitigate street activity. Work in close partnership with substance misuse and alcohol support teams to ensure individuals struggling with addiction can access necessary treatment. Proactively ensure that individuals have access to continuous healthcare services. Engage in ongoing statistical evaluations of individuals experiencing homelessness and participate in necessary street counts.

About You:

Proven experience in one or more of the following areas: substance misuse, alcohol misuse, mental health. Experience working with individuals facing multiple and complex challenges. Knowledge and experience in collaborative working across agencies, including facilitating multi-agency meetings. Strong communication, negotiation, and advocacy skills. Ability to establish and maintain relationships with a diverse range of services. Capable of working independently as well as collaboratively within a team. Proficient in IT, including the use of MS Office applications. Ability to critically assess and reflect on interventions, service delivery, and areas for improvement.

Please note that this role is not eligible for a Health and Care Worker visa. However, you may qualify for sponsorship under the Skilled Worker route, provided you meet the minimum salary requirements for the position.
