Clinical Case Manager

We are seeking a highly skilled and experienced Clinical Case Manager to join our team at Childrens Complex Care. As a key member of our healthcare team, you will be responsible for managing a caseload of children and young people with complex healthcare needs, providing high-quality care and support to ensure the best possible outcomes.
Key Responsibilities- Clinical Case Management: Safely manage a caseload of children and young people with complex healthcare needs, assessing, planning, implementing, and evaluating evidence-based care plans.
- Care Coordination: Collaborate with professionals across agencies and sectors to ensure safe and effective service delivery, working closely with families and other stakeholders to achieve the best possible outcomes.
- Training and Development: Provide essential training and support to staff working in packages, ensuring safe care delivery through our in-house Education Pathway.
- Clinical Governance and Risk Management: Ensure nursing practice is in accordance with the NMC Codes of Conduct, clinically accountable and responsible for delivering safe, effective, and evidence-based care.
- Staff Support and Development: Carry out staff appraisals and supervisions for registered nurses and Healthcare Assistants, educate and support non-registered healthcare staff on clinical delegated tasks.
- Collaboration and Communication: Effectively engage and collaborate with ICBs and external MDTs, ensuring seamless communication and coordination of care.
- UK registered nurse (RSCN or RGN) with strong clinical, management, and communication skills.
- Experience in managing complex healthcare needs and providing high-quality care to children and young people.
- Ability to work collaboratively with professionals across agencies and sectors.
- Strong training and development skills, with experience in providing education and support to staff.
- Excellent communication and interpersonal skills, with the ability to work effectively with families and other stakeholders.
