About the Position:
Lead and coordinate a solution-focused strategy, engaging Service Users in suitable services to transition them away from homelessness. Identify and connect with individuals experiencing homelessness and begging, providing immediate assistance through outreach efforts at various times and locations, including those reluctant to engage with services. Manage a caseload of clients with complex needs, creating tailored action plans aimed at reducing instances of homelessness. Collaborate in multi-agency initiatives designed to minimize street activity. Work closely with substance misuse and alcohol support teams to ensure individuals struggling with addiction can access necessary treatment. Proactively ensure that Service Users have access to continuous healthcare services. Engage in ongoing statistical assessments of individuals experiencing homelessness and participate in street counts as needed.About You:
Proven experience in one or more of the following areas: substance misuse, alcohol misuse, mental health. Experience working with individuals facing multiple and complex challenges. Knowledge and experience in multi-agency collaboration, including facilitating multi-agency meetings. Strong communication, negotiation, and advocacy skills. Ability to establish and maintain relationships with a diverse range of services. Capable of working independently as well as part of a team. Proficient in IT, including the use of MS Office applications. Ability to critically assess and evaluate interventions, service delivery, and areas for improvement.What We Offer:

Note: This role is not eligible for a Health and Care Worker visa. Candidates may be eligible for sponsorship under the Skilled Worker route, provided they meet the minimum salary requirements for the position.
