About the Position:

Lead and coordinate a solution-focused strategy, engaging individuals in suitable services to transition them away from homelessness. Identify and connect with individuals experiencing homelessness and those in need, providing immediate assistance through outreach efforts at various times and locations, including those hesitant to engage with available services. Manage a caseload of individuals with intricate needs, creating tailored action plans aimed at reducing instances of homelessness. Collaborate in multi-agency efforts to diminish street activity. Work in conjunction with substance misuse and alcohol support teams to ensure individuals struggling with addiction can access necessary treatment. Proactively ensure that individuals have access to continuous healthcare services. Engage in ongoing statistical assessments of individuals experiencing homelessness and participate in counts as required.

About You:

Proven experience in one or more of the following areas: substance misuse, alcohol dependency, mental health. Background in working with individuals facing multiple and complex challenges. Familiarity with multi-agency collaboration and experience in facilitating meetings among various stakeholders. Strong communication, negotiation, and advocacy abilities. Capability to build and sustain relationships with a diverse range of services. Ability to work independently as well as collaboratively within a team. Proficient in IT, including the use of MS Office applications. Competent in critically assessing and evaluating interventions, service delivery, and areas for improvement.

Note: This role is not eligible for a Health and Care Worker visa. Candidates may qualify for sponsorship under the Skilled Worker route, provided they meet the minimum salary requirements for the position.
