Advanced Clinical Pharmacist Cardiac Specialist

We are seeking a highly motivated and experienced clinical pharmacist to join our team in the cardiovascular and thoracic care group at University Hospital Southampton NHS Trust.
The successful candidate will work as part of a multidisciplinary team to deliver optimum patient care to CV&T patients, providing specialist pharmaceutical review of medication and advising on medication to optimise patient care.
The ideal candidate will have experience of cardiac patients, ideally with an independent prescribing qualification, and have completed or be nearing completion of further postgraduate study in clinical pharmacy.
This is a full-time band 7 clinical pharmacist position, working in a department of over 300 pharmacy staff. The successful candidate will be expected to work 1 in 5 late nights until 7pm and 1 in 6 weekend days (either Saturdays or Sundays).
Key Responsibilities:- Provide specialist pharmaceutical review of medication for cardiology, cardiac, vascular and thoracic surgery patients.
- Work with the multidisciplinary team to advise on medication for CV&T patients to optimise their care.
- Deliver education and training to a wide range of healthcare professionals working within CV&T.
- Contribute to department-wide audit, research, risk management and clinical governance.
- Develop guidelines to ensure safe prescribing practice.
- Work with clinicians to optimise medication for heart failure patients and assist with the development of specialist clinics for optimisation of secondary prevention medicines.
- A pharmacist with experience of cardiac patients.
- Ideally already have an independent prescribing qualification, but if not, we can support you training to become an independent pharmacist prescriber.
- Already completed or almost finished further postgraduate study in clinical pharmacy.

- Postgraduate diploma in clinical pharmacy or equivalent experience
- MRPharmS or equivalent
