Position Overview

The successful candidate will take a leading role in providing care to patients with designated chronic health issues, focusing on treatments, preventive measures, screening, and patient education. The aim is to deliver high-quality, evidence-based care that addresses the health and social requirements of the patient population.

Key Responsibilities

The role involves working autonomously as a Practice Nurse specializing in the management of chronic conditions and treatment room procedures within the professional scope.

1. Establish and manage clinics dedicated to chronic condition management, including but not limited to asthma, COPD, diabetes, and heart disease.

2. Develop and oversee care programs for patients with chronic conditions, ensuring adherence to local and national guidelines, protocols, and patient group directions.

3. Exhibit excellent communication skills to effectively engage with patients, taking comprehensive histories and fostering therapeutic relationships.

4. Assess, plan, and implement individualized care plans, referring patients to other healthcare professionals as necessary for specialized treatment.

5. Conduct routine health assessments and evaluate care outcomes.

6. Perform clinical investigations and utilize core competencies, including initiating diagnostic tests.

7. Recognize abnormal vital signs and respond appropriately.

8. Carry out investigative procedures such as ECGs, urinalysis, blood glucose monitoring, and venepuncture.

9. Demonstrate proficiency in examination techniques relevant to chronic condition management.

10. Promote health improvement by providing lifestyle advice and education.

Communication and Collaboration

1. Effectively communicate as part of a multidisciplinary team, liaising with various healthcare professionals.

2. Support the primary care nursing team and collaborate with administrative staff regarding patient information.

3. Manage communication barriers effectively to ensure clear information exchange.

Professional Development

1. Adhere to the Nursing and Midwifery Council's code of conduct and operate within the professional scope.

2. Lead the development of chronic condition pathways in collaboration with clinical leadership.

3. Participate actively in clinical meetings and ensure evidence-based practice is followed.

4. Maintain confidentiality and ensure accurate documentation of patient information.

5. Engage in continuous professional development, including mandatory training in risk awareness and safeguarding.

Leadership and Organizational Skills

1. Serve as a support network for colleagues, particularly within the nursing team.

2. Act as a role model and mentor for new staff.

3. Foster a culture that encourages effective chronic condition management and health promotion.

4. Ensure compliance with policies and procedures, adapting to changes in the workplace as necessary.

5. Participate in clinical governance and quality assurance initiatives to enhance care standards.

Qualifications

Essential:

  • Current NMC registration
  • First Level registration
  • Evidence of recent professional development
  • Training in wound management and compression bandaging

Desirable:

  • Relevant vaccination and immunization training

Experience

Essential:

  • Minimum of 2 years post-registration experience in a relevant field
  • Demonstrated good practice
  • Experience working autonomously in a nursing role
  • Experience in primary care settings

Desirable:

  • Experience in case management
  • Experience in sexual and reproductive health investigations/referrals
  • Familiarity with Systm1
