About the Role

We are seeking a highly skilled and compassionate Respiratory Support Worker to join our multidisciplinary team at Cornwall Partnership NHS Foundation Trust. As a key member of our team, you will play a vital role in delivering high-quality care to patients with chronic lung disease and breathlessness.

Key Responsibilities
  • Provide skilled support to patients in their homes, clinics, and community settings, promoting independence and self-management of their condition.
  • Work closely with GPs, acute hospital sites, and other healthcare professionals to ensure seamless care coordination and delivery.
  • Develop and implement individualized care plans, incorporating pulmonary rehabilitation programs, exercise classes, and education sessions.
  • Conduct assessments, use various treatment skills, and evaluate patient progress, reporting back to qualified clinicians as needed.
  • Manage a caseload of patients, implementing agreed management/care plans and providing ongoing support.
  • Collaborate with the team to develop and deliver education programs, promoting patient empowerment and self-management.
  • Participate in multidisciplinary meetings, contributing to the development of care plans and strategies.

Requirements
  • Experience of working as a Support Worker in a healthcare setting, preferably in a multidisciplinary environment.
  • Relevant experience in an appropriate care setting, with a strong understanding of respiratory care and management.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work independently, with minimal supervision, and as part of a team, demonstrating a flexible and adaptable approach.
  • Hold a valid UK driver's license and be willing to travel to various locations, including patients' homes and community clinics.
